I have read and agree to follow all playroom rules and regulations. I understand that failure to follow these rules may result in removal from the facility without refund.

I hereby release and hold harmless M&M Playland Café, its owners, employees, and agents from any and all liability, claims, demands, or causes of action arising from my child's participation in activities at the facility. I understand that use of the playroom carries inherent risks, and I assume full responsibility for any injury or damage.
